Tautoko Hauora

Tautoko Hauora is a service that provides support to individuals who are either not enrolled in a general practice or who are enrolled with a Pegasus practice but not accessing their general practice teams as frequently as required.

Tautoko Hauora service supports Tangata whai ora (people seeking wellness) who are not enrolled in General Practice, or who are enrolled with a Pegasus General Practice, but need additional support.

Kaitautoko (support workers) from the Tautoko Hauora service can support Tangata whai ora to access primary health care and social services in Canterbury. Kaitautoko can carry out home and community visits when appropriate.

If the Tangata whai ora is enrolled with a GP, and the General Practice has a Health Improvement Practitioner (HIP) or health coach, the Tangata whai ora may be referred back to the General Practice.

An assessment of Tangata whai ora and a plan around health and wellbeing will be carried out. The Tautoko Hauora service provides a brief intervention and referrals can be made to other health services as needed.

Kaitautoko can support Tangata whai ora by:

  • supporting access and referral pathways to health care and social services
  • navigation of the primary health system
  • helping to identify needs impacting on health
  • providing information about supports/resources available in the community and connect Tangata whai ora with community support services.

Please note: Tautoko Hauora is not an emergency service.

Criteria

Tangata whai ora with a community services card or a Work and Income client number.
