    How to get there
    We are located at 401 Madras Street, Christchurch Central.

    Parking
    You can enter our carpark from Madras Street; turn left just before the lights on Bealey Avenue. We have a drop off area at the front entrance for patients who may require this.

    Public Transport
    You can plan your bus trip from the Journey Planner on the Metro website.

    Accessibility
    We have wheelchair parking and an accessibility ramp. Wheelchairs are available if you need them. We also have an interpreter service available.

As a service provider or supplier to Pegasus Health and 24 Hour Surgery, your services support us to deliver quality health care to communities throughout Ōtautahi Christchurch and Waitaha Canterbury.

We are committed to creating a safe working environment for all people working at Pegasus House, and 24 Hour Surgery, including you and your team.

Before you begin work as a contractor, you need to review the procedures and complete the health and safety questionnaire below. This also needs to be completed every two years.

Step two: complete the health and safety questionnaire

The form will take 20 minutes to complete. Please collate the information you will need before you begin, as you will need to complete the form in one session as you cannot walk away and return to it later, as the form may time out. Before you begin

The form covers the following information about your organisation, so you’ll need to collate this before you begin:

  • Role and individual who is responsible for health and safety at your organisation
  • Health and safety record keeping and policies
  • Health and safety procedures
  • Training and supervision
  • Emergency procedures
  • Management of risks and hazards
  • Reporting and investigation of accidents, incidents, and illness
  • Use of hazardous substances
  • Testing and maintenance of plant and equipment
  • Monitoring, measuring, and improving health and safety
  • Insurance – including policy details
  • Use of subcontractors.
