Partnership Community Workers

If you need help finding health care, Partnership Community Workers* (PCWs) can help you:

  • enrol at a Pegasus Health general practice
  • get to your doctor/hospital appointments, if transport is a problem for you
  • visit Work and Income, if you find it difficult to pay for the doctor
  • find other Community Services to support you.

*PCWs are only available for those enrolled at a Pegasus Health general practice. If you are not enrolled with a general practice, PCWs can help you enrol with a Pegasus Health general practice.

How to contact a PCW

Talk to your general practice about getting help from a PCW.

If you don’t have a regular GP, call 03 379 1739 and asked to be put through to the PCW team or use the PCW contact list to find a PCW near you.

How to refer someone to a PCW

If you are a community provider or GP who has a client, or there are people within your community who are in need of medical help but are unable to access this help for various reasons, please use the Referral Form and information on this page.
